FOR THE PROVISION OF DOCUMENTS FOR MEDICAL ENTITLEMENT OF THE MINOR

It is most respectfully submitted on behalf of the applicant above named that the minor is entitled to free medical services which is offered to the employees and their families by the employer of the applicant. Hence, this Honourable Court may kindly be pleased to grant the permission to the applicant to receive the ORIGINAL B FORM & Birth Certificate of the minor which is mandatory for medical entitlement of the minor. This is for the sole purpose of the welfare of the minor.

Further that it is pertinent to mention here that the application for same purpose was filed previously on behalf of the applicant. However, the respondent did not make proper compliance of the said order due to the malafide intentions of the respondent.

The prayer is made in the larger interest of justice equity and good conscience.
